Caregiver access to a patient's online medical record

Patients who have an active MyChart account or create a MyChart account may share their record with another individual.

Click here for the instructions on how your loved one may share their record.

Once you have accepted and are granted access, you can view their record in MyChart or MyChart Bedside (available during some hospital stays).

I understand that:

• To use MyChart to access the patient’s record, I must log in with my own username and password.
• To access the patient’s record through MyChart Bedside, I must log in with my own PIN (set up with hospital staff).
• I agree with the terms and conditions.
• In an emergency, don’t use the portal.
• Access is revoked when:
• The patient removes your access within their own MyChart account.
• Communications on the patient’s behalf must be sent from their record. Alerts will be sent to the email address on file.
