Power of Attorney access to a patient’s online medical record

With documented Power of Attorney (POA) access, you can view your loved one’s medical record using MyChart or MyChart Bedside (available during some hospital stays).

Just complete the form and follow the instructions. Include a copy of your POA legal document if it’s not already on file. Once approved, we’ll send you a message in MyChart. We try to process requests within 3 to 5 business days of receiving the signed form, but it may take longer. We’ll process your request as soon as possible.

Requirements for access:

  • Power of Attorney documents on file
  • Completed POA authorization form
  • Each person requesting POA access must have their own account, including for MyChart Bedside

If your POA is revoked or limited, report the change to your healthcare organization.

I understand that:

  • To use MyChart, I must log in with my own username and password.
  • To use MyChart Bedside, I must log in with my own PIN (set up with hospital staff).
  • I agree to the terms and conditions.
  • I’ll lose access when my POA rights expire due to the patient’s death, language in the POA that causes termination before the patient’s death or the patient revokes the POA.
  • In an emergency, I shouldn’t use the portal.
  • POA access is revoked when the patient or physician submits a request or revokes it online. Your healthcare organization reserves the right to revoke at any time.
  • Communications on the patient’s behalf must be sent from their record. Alerts will be sent to the email address on file.
