Do you have more than one health insurance plan? Submitting the coordination of benefits form allows us to help you manage your multiple plans and keep your information up-to-date. This helps us maximize your benefits and reduce your out-of-pocket costs.
Complete this form if you have a health insurance plan other than Geisinger Health Plan or if you have received a letter from the plan's coordination of benefits department.

- Your policy and group number for your other plans. Your Medicare number (if you have one).
