Our care managers work closely with you to provide support and education for common health conditions and social issues. They can help you to:

  • Understand how to eat healthier and exercise
  • Develop healthy habits and make lifestyle changes
  • Pay careful attention to medications, follow-up appointments with providers and home health services
  • Ensure continual monitoring of your condition
  • Recognize when your condition may be worsening and when to contact your doctor
  • Transition from hospital to home or a skilled nursing facility

Your health benefits cover the cost of care and tools provided. Coronary artery disease
Coronary artery disease includes heart disease, stroke and poor circulation in your legs. It can occur as a result of many other things, including high blood pressure, diabetes, high cholesterol and family history. The key to managing coronary artery disease is diet and exercise. Your care manager will:
  • Get tools to better understand the role of sodium and fat in your diet
  • Work with your provider to best use your medications to control your cholesterol, blood pressure and blood sugar
